this post is 3 years old, VAC changes things up almost monthly, this form may no longer be in use. I know I read they shortened a disability form package taking out most of the mental health questions that a dr. You make a good point there By The Bay. VAC wants to control who can apply for these benefits, so that they don't get flooded with requests. So with DEC becoming more and more of a known issue, I would not doubt that it eventually changes so that the paperwork can only be requested by a Case Manager. I was a VAC client for several years, before I even heard the term TPI, and it meant you could not work, at all, So not many Veterans qualified. But with DEC, where it has more varying degrees, (the ability to earn more or less than 66.6% or 33.3% of previous income), far more Veterans fall under it's umbrella. And since it is used to determines the level and duration of benefits that a Veteran can receive, it's become a very important variable that Veterans are trying to, rightfully, take advantage of.

Hi LostTrucker,
Unfortunately, the copy that I have can only be used as a reference because My VAC Case Manager filled out portions of the form with my personal information making it unuseable for others to use it. It even has a bar code which maybe specific to each veterans' file.
VAC671 forms are not made available through My VAC Account. You will need to request a copy to be sent to your doctor. You can make the request through My VAC Account or phone your local VAC office to mail out a copy. I think you would be hard press to find a blank copy just floating around out there. VAC keeps that info locked up tight. They just had to make it difficult for veterans to access it. I guess VAC wants to have the controlling power over how the form is processed.

Yes, it is the same form for both Mental and physical disabilities.

EMPLOYMENT CAPACITY - VAC 671
I have a question with respect to form VAC 671 and being determined DEC. I have a great CM and she is bringing forward my file for a DEC determination. She will be asking my family physician to fill out form VAC 671 to be included in my DEC determination. I am on private disability insurance with GWL and they have determined that I am disabled with respect to medical and functional assessments, therefore, I shall continue to receive my disability insurance. I am also 109% DA with other ailments that have been approved but no % assigned as I am already over 109%. I can't find this VAC 671 form online and I am a bit concerned as to what my family physician will be required to report on me and I won't be there update him on my signs and symptoms. With all of this said can anyone provide insight as to my chances of being approved DEC? I am also 59 years old.
