The receptionist got me to fill in some details on the app that I had apparently missed, and said the Dr. has done tonnes in his 20+ year career.
OK. So just send in what he gave me? or send the reports in as well?

If your physician filled it out correctly, you should be good to go.
Your physician should have or be aware of those assessment reports to aid in filling out the application.
If they require more info, they will notify you or your physician.

The notification just reminded me that I need to send in my DTC app. My Psych filled it out, but there is no assessments attached... should I include the assessments and reports he did for VAC?

I just wanted to follow up to see how Firebird had made out. With tax season upon us, did you get your DTC in place? It seems with all the hoops and steps in play it is a years process.... oh joy!

The wait time after all paperwork has been sent in is 3 - 6 Months, depending on their workload.

After you send Form T2201 – Step by step process
Step 1 – Form T2201 is received at CRA
The CRA receives the fully completed and signed Form T2201, Disability Tax Credit Certificate.
Step 2 – Application is reviewed
The CRA will assess your application to determine if you are eligible for the disability tax credit (DTC) based on the information provided by your medical practitioner. If we need more information, we may contact you or your medical practitioner.
If you sent your Form T2201 with your income tax and benefit return, we will review your application before we assess your return.
Note: You can now submit supporting documents online through My Account. To use this service, you must have received a letter inviting you to do so.
Step 3 – CRA’s decision
We will send you a notice of determination to inform you of our decision.
Application is approved
The notice of determination will identify which year(s) you are eligible for the disability tax credit. You won't need to submit a new application every year, unless we tell you that we need one. Information about other programs that are dependent on eligibility for the DTC may also be included in the notice of determination.
You can view your DTC information in My Account.
You should tell us if your medical condition improves to the point that you no longer meet the criteria for the DTC.
Application is denied
The notice of determination will explain why the application was denied. Check your copy of Form T2201 against the reason given, since we base our decision on the information provided by the medical practitioner.
If you disagree with our decision and have additional information from a medical practitioner that we did not have in our first review of your application, send it to the Disability Tax Credit Unit of your tax centre. We will review your file again using the new information.
You also have the right to file a formal objection to appeal the decision. The time limit for filing an objection is 90 days after we mail the notice of determination.
Step 4 – Claim the disability amount
The notice of determination provides instructions about which year(s) you can claim the disability amount on your income tax and benefit return.

I simply found the form on line printed it and took it to my psychologist. There is a block on the form that asks the Dr. to explain how his patient meets the requirements and instructed to provide greater detail on a separate sheet of paper if need be. He send the form and the letter to my local city taxation office, not the main CRA office in Ottawa ( if there is one ). It only went out this week but I have a feeling it will come back to me as having been sent to the wrong location, after some clerk has read everything about me of course.

I was wondering the same thing about privacy.
I still havent gotten around to getting my DTC filled out. Havent done my CPPD either :S
i know I need to do it, motivation is low and it all seems overwhelming.
I honestly havent even looked at the applications in depth.
Do they take a lot of time or effort?
