Contact Veterans Affairs Canada at 1-866-522-2122, ask them to send you an application for Hearing Loss. http://www.veterans.gc.ca/eng/services/after-injury/disability-benefits/benefits-determined/entitlement-eligibility-guidelines/hearing_loss
Once you receive the application, fill it out with the required info from the application.
