Re: SISIP Annual Medical Forms (Assorted Topics)
Personally it does not bother me to have and annual medical for continued SISIP benefits, as I see my doctor often, but why the change from the initial way of change, if you are already on CPPD you just have to fill out the members form annually, I just spoke with SISIP two Weeks ago and the individual who sends out the medical forms, stated and or confirm that if you are on CPPD , you will only have to fill out the members form, in my view if and individual has been on CPPD for let say 10 + years, why make their family doctor fill out the forms yearly, those that don't have access to a Family doctor will have undue stress placed upon them, something must have happened to make them change their mind. I am not complaining, I just feel for those who are so ill they can't make it in to their doctor, or as stated above don't have a doctor. I do understand the need for all members who were reinstated to have a 2013 medical update. Anyhow there must be some Legal issues that caused them to keep the status quo.

tiger I think you should call the phone number ( 1-800-565-0701) and talk to your CM about this situation. Your problem is way above my pay grade and I don't want to pass on poor info to you.
Make sure you tell them that you don't have a family doctor. I'm sure that with our wonderful healthcare system here in Canada that you are not the only one without a family doctor. Also ask if Manulife is going to update their forms.

Re: SISIP Annual Medical Forms (Assorted Topics)
Teentian, maybe you can help on this. I received my package, I took the medical questionnaire to the Doctor at the CLSC in Québec because I don't have a family doctor. I had with me the SISIP letter and a copy from the RRQ stating that I am invalid. The doctor looked at all the paperwork and said that he could not fill out the form as he does not have a copy of all my medical records. He also stated that he cannot understand why SISIP wants this form sign when they have all the paperworks stating that I am invalid. and that I will not get better, like he said, you are not invalid one year, better the next year and invalid again. There is a good reason why I was declared invalid and SISIP have all that paperwork. I have e mailed SISIP twice on this asking them what I should do and they never got back to me. If all the want is a medical update, then they should change their form, because the form that they have right now is the same for when you first apply for SISIP. It is not clear as mud, to get medical paperwork sign when you don't have a family doctor. thank you.

Re: SISIP Annual Medical Forms (Assorted Topics)
This is the correspondence I received today from SISIP.
In regards to the scheduled annual medical review, I'll make note in your file that the Attending Physician's Statement will be delayed. I will follow up with you around the end of February 2014 if I don't receive it by then. In the meantime, I will ask you to please complete the Application for Continued Benefits (Claimant Statement) and forward it to our office.
Now that I know you've applied for CPPD (Canada Pension Plan - Disability Benefits), I will now be sending you forms (Authorization to Communicate Information Service Canada). Please read over these forms and contact me with any questions or concerns. These forms will also prompt Service Canada to request medical information (with your consent, of course) from your SISIP LTD file with us to help make a decision on your CPPD claim. That said, I am also forwarding a copy of the letter you requested supporting your total disability under our definition.
Finally, concerning the Claimant Statement (ML 1OE (01/07) Page 2 of 2 para J.) - please disregard this question; we have not updated our forms since the VAC disability offset ceased.
Please call me directly anytime if you have any questions.
Now seriously does this sound like a bunch of insensitive morons, absolutely not I was actually impressed with the response time less than 1 business day to give me validation and the required info to ease some of the stress in living in the boonies and having doctors that cannot see you tomorrow.
Finally I can say thanks SISIP
THANKS

Re: SISIP Annual Medical Forms (Assorted Topics)
TeenTitan is in fact correct & to add The Sisip Dept. would simply like to gather all information they can regarding your claim to ensure a smooth
process for future payments , so the more information your Doctor can supply each & every year simply puts a stronger foundation under you to support your monthly LTD Payment for years to come , ensuring that you are still entitled across the board .
.......... SO TROOPS , make sure to see your family physician , phyciatrist ,
or psychologist as often as necessary & have it all documented as required
for annual sisip reports to follow .

Annual Medical Records
I was contacted by a Snr. CM from Manulife in Halifax to pass on the following info.
As we know we are all currently receiving a request from SISIP to update our medical records for LTD. It was stated last month that LTD clients on CPP/QPP Disability would only do a yearly questionaire from SISIP after updating their medical records this year.
SISIP is going to follow the policy that has always been in place. All LTD clients will be required to have their doctor fill out the forms to keep our records up to date with SISIP every year. And to make sure a LTD client is seeing a doctor/specialist for their injuries.
When your doctor submits the paperwork this year the month it is received by SISIP will be when you get the same request the following year. So if SISIP receives the report from your Doctor in December you will get the same request next December.
If you have any questions for your CM call 1-800-565-0701

Re: SISIP Annual Medical Forms (Assorted Topics)
Good morning all. Haven't received my LTD forms yet on the island. I'll wait a week then get on it. Sisip knows where I am hiding. I hope everyone is doing well and awaiting christmas with anticipation. My kids are coming home in 8 days!! Going to be a great visit in our new location. Hobbling down the beach just doesn't get old here. Keep well and keep looking out for each other. That is what this site is great for. We have come a long way on our journey, but we have many more obstacles ahead. Have a great day!

Number One Rule: Never try to figure out GOC as the word "logic" rarely applies. Many times the options of keeping offices open, closing offices, contracting out, eliminating duplicate services etc. are made based on some political hack determining voter impact in a specific or general area. Sorry I have been absent for awhile. Just completed a 5 week Pain Management Course at Miller Centre here in St. John's. Super bunch of staff, from Doc, Occupational Therapist, Physiotherapist, Nurse, Dietician, Social Worker and Phychologist.

Re: SISIP Annual Medical Forms (Assorted Topics)
The reason I got the call Navrat is people currently under the LTD program called and asked
"What is going to happen to me? I'm in the middle of my voc rehab? Who do I report to?"
That's why I got the call Navrat. Some people panicked and got very worried.

Re: SISIP Annual Medical Forms (Assorted Topics)
Rex the whole point of my post is what Nav posted was a rumor. There is no study. The AG and OVO suggested a study be done. That's it. A suggestion.
It is also more complicated then you think to just plug laid off VAC employees into doing vocational rehab. And the GoC still has to pay salary, pension, benefits whereas SISIP is a contract and Manulife pays the SISIP employees.
Look at it this way if it was as easy as not renewing the SISIP contract and let VAC take care of all the voc rehab wouldn't they have done by now?
I'm just asking everyone be careful when posting a rumor because that is all it is...a rumor. There is zero truth to what Navrat said. If you want to prove there is truth to it Navrat ask the guy who told you this rumor to come out and tell/show us his facts.

Re: SISIP Annual Medical Forms (Assorted Topics)
None taken by me, Teen, I am wondering why anyone would call if there wasn't some truth to this, or it's being discussed, nevertheless I won't repeat " Rumours" from the legion anymore! I'll wait until Dear Leader releases it
