I have received two T4A's and one T5 from Manulife.

I used my COLA report from MC given in June 2015 for my income tax return.

I scratched out my name on my extra t4a slip and wrote in Trooper C/O CSAT! Hope you don't mine Troop!

Sparrow the T5 for interest for sure.

The T1198 has some guidelines with respect to the type of funds received an the amount of funds received.
So I'm not so sure if everyone will get a T1198 ...if any at all.

You can contact your CM an ask about both the T5 & T1198 ...just to be on the safe side.

Received two T4A's today from Manulife. So I need to still receive a T5 and Form 1198 correct??

My COLA amount that was given to MC was around $270 and I just got a T4A from Manulife for $10.78. LOL.
